KVKK Application Form
- General Explanations
In the Law No. 6698 on the Protection of Personal Data (hereinafter referred to as the "KVK Law"), the personal data owner (hereinafter referred to as the "Data Owner"), you may apply to VLM 4 YAPI SİSTEMLERİ YATIRIM A.Ş. (hereinafter referred to as the "Company") as the Data Controller based on your rights listed in Article 11 of the KVK Law.
- Application Method
Pursuant to paragraph 1 of Article 13 of the KVK Law and Article 5 of the Communiqué on the Procedures and Principles of Application to the Data Controller, your requests regarding your rights hereunder must be submitted to our Company in writing by printing out this Application Form with one of the methods described below or other methods determined by the Personal Data Protection Board (hereinafter referred to as the "Board").

In the application you will make to exercise your rights as a personal data owner and containing your explanations regarding the right you request to exercise; If someone else is making the application acting on your behalf, you must document the authority of the person who will make the application in this regard.
Pursuant to paragraph 2 of Article 13 of the KVK Law, your applications submitted to us will be answered "as soon as possible and within thirty (30) days at the latest" from the date of receipt of your request, depending on the nature of the request. Our responses will be delivered to you in writing or electronically in accordance with the provision of Article 13 of the relevant KVK Law.

- Declaration of the Applicant
This Application Form has been issued in order to determine your relationship with our Company and, if any, to respond to your personal data processed by our Company accurately and within the legal period. In order to eliminate the legal risks that may arise from unlawful and unfair data sharing and especially to ensure the security of your personal data, our Company reserves the right to request additional documents for identification and authorization. In the event that the information regarding your requests submitted within the scope of the Application Form is not correct and up-to-date or an unauthorized application is made, our Company does not accept any liability for the requests arising from such incorrect information or unauthorized application.
